Contains records on the early ecclesiastical history of Provo, Utah. Records include accounts of sermons of the period by several prominent Church leaders during the Mormon Reformation and document the response of the community, including rebaptisms and Church disciplinary matters. Also includes Blackburn's personal family record, and records of donations by Loa (Utah) Ward members for the building of the Manti Temple during Blackburn's tenure as bishop. Dated 1851-1887.

Ledger was later used from 1888-1893 for meetings of a committee to build a duck pond, later called the St. George Fish Pond Company, for which Carter was president and which was on his property. Contains minutes of meetings of the high priest's group in Joseph, Utah between May and June 1894. Also includes a record of their "teams," which could possibly be teams of livestock, in 1896. Also includes brief account of a hunting trip Leavitt had in Southern Utah possibly in 1896, including how many rabbit and quail were killed each day. In all, the dates of the contents range from approximately 1894-1896.
