Hard cover, black binding bordered by a red spine and corners. The word "Record" is printed on the spine in gold. This volume consists of 196 pages with printed pagination in blue ink at the upper corners of the page.

A note on the title page of the book, added by a later author, reads "Book A Patriarchal Blessings Given By O. B. Huntington Jr. Commencing 23 February 1926 ending 25 September 1928. I have never seen the record of Blessings from Sept. 1928 to Aug. 1930. Nearly 2 yrs. As far as I can learn the Blessings from Books A & B are recorded in the Historian's Office in S. L. Many Blessings are not here nor in Book B & many seem to not be in the Historian's Office Either. Mattie Hiatt. This Book contains 63 Blessings."
