Scope and Contents Report by Kristee A. Roach, who collected information from Richard Roach. Several high school students hear reports of a monster in Payson Canyon with multiple eyes. They go to look for it and find that it is a dummy with reflectors sewn on, giving the appearance of eyes in the dark.

Scope and Contents note Report by Utah State University folklore student Jensie L. Anderson, who collected information from Teresita Gonzalez. A group of friends go out to a rundown shack on the edge of a farm. They see three people surrounded by green light: a man, woman, and child. The man uses an ax to cut the woman's head off and the group runs for it. One of them looks back, but the man and child have disappeared. The shack apparently once belonged to an overseer who killed his wife and child.

Scope and Contents note Report by Brigham Young University folklore student Linda Hampton, who collected information from Brian Sutcliff. A soldier screams through the night, claiming he is seeing a purple monster. MPs finally take him to a cell to allow the other soldiers to get some sleep. After three days he stops screaming and they find him in the corner of the cell with teeth marks on the back of his neck.

Scope and Contents note Report by Utah State University folklore student Rob Bartlett, who collected information from Doug Davis. Outside of Buhl there is a four-way stopsign with a house on its corner. Several years ago someone ran the stopsign and caused an accident which claimed the lives of four out of five family members living at the house. Now whenever someone runs that stopsign, a red light apears in the house as a warning. If the same person runs the stopsign again, they will be killed.
