Typewritten form letter, dated 12 Dec. 1901, and signed by Barton O. Aylesworth. The item was sent to George S. Burleigh requestiong biographical information. Burleigh answered the questions on the form letter and returned it to Aylesworth. Also included is the envelope in which the item was sent.
